Evidence of Authority

Now we need to determine whether you will need to include an Evidence of Authority in your agreement. Generally you will not have to submit an Evidence of Authority if you receive notices under only one name and on your behalf only.

NOTE: You are eligible to receive notices electronically if you are named as a Trustee, Attorney, Creditor or Debtor on a Notice that you would otherwise receive from the Court via the U.S. Mail.

An Evidence of Authority is required if you answer "YES" to any of the following questions:

  1. Are you submitting names & addresses based upon multiple related company names ( i.e. parent or subsidiary, or attorneys at a law firm) ?

  2. Are you submitting a list as an agent of another company (i.e. a third party) ?
    YES
    NO
